COMMUNITY HEALTH REPRESENTATIVE
Brief desription:
The CHR will be under the direction and supervision of the Health Manager and will provide health cate support services to community members in coordination with Healthcare Professionals to achieve, community healthcare objectives.
Tasks:
The Community Health Representative will collaborate with the Community Services Health team in ensuring that all members’ health needs are addressed. They will plan, coordinate, and implement all the health promotion activities for the community.
· Encourage and support community initiatives and activities to facilitate greater community education, responsibility, and involvement in health promotion.
· Will be knowledgeable on health services and benefits provided by Apitipi Anicinapek Nation, FNHIB, Health Canada.
· Support and advocate on behalf of members with external agencies (e.g., NHIB and Health Canada) and with local health care providers.
· Oversee and assist the Health Program assistant in delivering the medical transportation benefits program. Prenatal and Healthy Baby Program.
Qualifications and Requirements:
Grade 12 or equivalent with health education background relevant to the position
· Completion of Aboriginal Health Community Administration Program certificate (UBC) preferred.
· Demonstrate a healthy lifestyle and being a positive role model for the community
· Experience working with Electronic Medical Records (EMR) programs an asset · Strong people skills; ability to establish and maintain effective and efficient working relationships with clients, their families, management, and staff.
· Ability to work as a team member as well as self-directed. · Experience with Microsoft Word and Outlook · Ability to write or complete required reports · Good verbal and written communication skills.
· Work with the Health Nurse to deliver programing and assistant to the nurse.
· Valid driver’s licence, and use of a reliable vehicle for work-related responsibility
Competencies (in order of importance)
· Ability to deal with people sensitively, tactfully, dipolomatically at all times
· Ability to remain calm and poised in urgent situations
· Clean criminal record check
· Valid driver’s license with a clean driving abstract
· Grade 12 Diploma
· Community Health Representative (CHR) certificate preferable, or willing to obtain.
· Two (2) years’ experience would be an asset.
· Clean vulnerable sector check
· Ability to effectively communicate both verbally and in writing.
· Ability to work individually, as well as part of a team.
· Ability to work under extreme pressure and handle tense and stressful situations.
· Demonstrate time managements skills.
· Elevated level of integrity and work ethic
· Strong senses of ethics and the ability to manage sensitive or confidential information with tact and direction.
· Valid First Aid and CPR certification, or willing to obtain.
